For privacy reasons the Fair
Credit Reporting Act and the Driver Protection Act
places certain restrictions on Driving records as well
as other consumer reports. Both require a permissible
purpose before ordering reports. The FCRA places
further restrictions on the use of reports when they
are used for employment purposes.
To order reports form this web site we require a
signed Subscription Agreement in which you certify
that you have read and understand and will abide by
the Fair Credit Reporting Act and Federal Driver
Privacy Protection Act as well as other federal and
state laws when using records from this site.
Reports are paid for using a debit account system.
You will be required to initially fund your account
with a minimum of $50.00. This money will be placed in
a non-interest bearing Escrow account. The cost of
each record ordered will automatically be deducted
from your account. You will be able to view your
account at any time and replenish the account as
needed. Records cannot be ordered unless available
funds are on hand.

If you don't already have The Adobe

You will need it to read the forms that we provide for
you.

To
Sign Up For Services:
- You must be a legitimate business and be the
end-user of the reports.
- Click Subscription
Agreement and print a copy from your browser.
Complete all blanks and sign the Agreement.
- Fax to 1-866-301-4391 or mail to Florida MVR
Services, Inc P.O. Box 13905 Tallahassee, Florida
32317.
- Include a copy of your business and any
professional license and a copy of a Federal Form
with your Business Name and Federal ID Number
imprinted.
- Fund your account by mailing a check made
payable to Florida MVR Services Escrow for a
minimum of $50.00, or use your Visa, MasterCard,
Discover or Amex card to
fund your account.
When approved, we will establish your account,
provide you with a User Name and Password, and you can
immediately begin using our system.
