understand that Florida MVR Services,
Inc. will purchase a copy of my
driving record which has my
name and address disclosed from the State DMV
and will make it available me usually the same
busines day. I understand this is not an Instant
Record and the cost for the record is
$30.00. |
| 2. I
understand that if the Driver License
Number is entered incorrectly and I
receive a "NO RECORD FOUND WITH
THIS DL NUMBER" report from the
State DHSMV I will still be charged
the full amount for the report. |
| 3. I
understand that Florida MVR Services,
Inc. obtains the driving record
directly from the Florida Department
of Highway Safety and Motor Vehicles
and the information received is that
which is currently available for the
type record I am requesting. I
understand that Florida does
not display a violation,
suspension, accident or other charge
on my three-year or seven-year Record
if I was found not guilty, or if the
charge was dismissed, or if
adjudication was withheld. I
understand that if I received a ticket
and then went to driving school to
have the points removed from my record
the infraction is considered
"adjudication withheld" and
will not appear on my three-year or
seven-year Florida record. I
understand that if I need this
information I must order a complete
record.
A complete driving record contains all driving
infractions and correspondence from the Florida
DMV for the past 11 years. If the Florida DMV
has received infractions reported by other
states it will include those infractions as well.
Certain serious infractions such as DUI's may
remain longer than eleven years. |
| 4. I
certify that the driving record I am
requesting is my personal driving
record and the information I provide
is my own. I understand that
requesting information of any other
individual besides myself can result
in state or federal enforcement
actions as well as private lawsuits.
In addition, I understand that any
person who knowingly and willfilly
obtains a consumer report under false
pretenses may face criminal
prosecution. |
